Records Clerk & Records Specialist- Work From Home

We are seeking an organized and detail-oriented Remote Records Specialist/Records Clerk to manage, maintain, and process patient medical records in a secure and compliant manner. This role supports healthcare providers, patients, insurance teams, and administrative staff by handling record requests, updating files, ensuring accuracy, and safeguarding confidential information in accordance with HIPAA regulations.

Responsibilities

  • Maintain and update Electronic  Records (EHR/EMR) systems accurately

  • Process records requests from patients, providers, attorneys, and insurance companies

  • Verify authorization forms and ensure all releases comply with HIPAA and privacy policies

  • Retrieve, scan, upload, and index medical documents into patient charts

  • Ensure records are complete, legible, and properly stored

  • Track and log incoming/outgoing medical record requests

  • Communicate with physicians, nurses, and staff to obtain missing information

  • Facilitate continuity of care by transferring records between providers

  • Assist with audits, data integrity checks, and quality control reviews

  • Respond to phone calls, emails, and secure messages regarding records status

  • Prepare and send medical records via secure methods (fax, encrypted email, patient portals)

  • Maintain confidentiality and follow strict data security protocols

Qualifications

  • High school diploma or equivalent

  • 1+ year of experience in medical records, health information management, or healthcare administration (varies by employer)

  • Knowledge of HIPAA, privacy laws, and release-of-information procedures

  • Proficiency with EHR/EMR systems (Epic, Cerner, Athena, eClinicalWorks, etc.)

  • Strong organizational and data-entry skills

  • Ability to handle confidential information with integrity

  • Excellent communication skills (written & verbal)

  • Reliable high-speed internet and private workspace

  • Ability to work independently with minimal supervision

  • Experience in release of information (ROI)

  • Previous remote work experience

  • Knowledge of medical terminology

  • Bilingual (English + Spanish or other languages)
